Job summary

Angels Care Hospice is seeking a driven Account Executive to generate referrals from physicians, hospitals, SNFs, and other healthcare partners. The role focuses on building strong relationships and promoting hospice services to ensure eligible patients receive quality care.

You will work across clinical and operations teams to achieve referral and admission targets, maintain CRM activity, and contribute to patient satisfaction and smooth transitions of care.

Qualifications

• Proven success in healthcare sales or post-acute care is required.
• Strong communication, presentation and interpersonal skills are essential.
• Ability to work independently and manage time effectively in a fast-paced environment.
• Valid driver’s license and reliable transportation are required.
• Familiarity with EMRs and CRM systems is preferred.

Responsibilities

• Develop and maintain relationships with referral sources across your territory (physicians, discharge planners, case managers, community partners).
• Promote hospice services, highlighting patient eligibility and seamless transitions of care.
• Identify new business opportunities and convert them into referral sources.
• Collaborate with clinical and operations teams to ensure high-quality service delivery and patient satisfaction.
• Attend IDT, case conferences and other meetings as assigned.
• Meet or exceed monthly referral and admission goals.
• Maintain accurate records in CRM systems and track outcomes.
