Job summary

Warm Hearth Hospice in Christiansburg, VA is seeking a motivated Hospice Marketing and Community Liaison to grow our hospice program across the New River Valley. You will develop relationships with hospitals, physicians, SNFs, and other partners to expand referral networks and educate the community about hospice services.

The role requires strong communication, networking, and presentation skills, with a patient-centered approach and ability to manage a territory independently.

Qualifications

• Experience in healthcare marketing, community outreach, business development, or relationship-based sales preferred.
• Hospice, home health, senior living, or other healthcare experience strongly preferred.
• Strong interpersonal, communication, networking, and presentation skills.
• Ability to establish and maintain professional relationships with healthcare providers and community organizations.
• Self-motivated, organized, and able to manage a community-based territory independently.
• Valid driver's license, reliable transportation, and appropriate automobile insurance.
• A degree in marketing, business, healthcare administration, or a related field is preferred but not required with relevant experience.

Responsibilities

• Develop and maintain relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, home health agencies, social workers, discharge planners, and other healthcare professionals.
• Identify new referral opportunities and strengthen existing community partnerships.
• Conduct regular outreach visits, educational presentations, and in-service programs.
• Educate referral partners about hospice services, eligibility, and the referral process.
• Respond promptly to referral opportunities and coordinate with the hospice clinical team to facilitate appropriate transitions of care.
• Represent Warm Hearth Hospice at community events, professional meetings, and networking opportunities.
• Develop and implement marketing strategies to increase hospice awareness and support sustainable program growth.
• Track outreach activity, referral trends, and marketing results.
• Collaborate with hospice leadership and clinical staff to ensure excellent communication and service for patients, families, and referral partners.
• Promote hospice services ethically and in accordance with applicable healthcare regulations.
