Job summary

St. Croix Hospice seeks a Care Transition Coordinator to build strong referral relationships and educate patients and families about hospice care.

You will coordinate admissions, manage CRM-driven outreach, and support the care transition team across multiple territories. The role requires strong communication, organizational skills, and the ability to work autonomously within a compassionate, patient-centered environment.

Qualifications

• Associate’s degrees in Marketing, Business Administration, Healthcare, or related field.
• Ability to position services to customers tactfully.
• Excellent communication, negotiation, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation.
• Ability to pass DHS background study.

Responsibilities

• Meet admission and call volume targets and coordinate daily sales and marketing operations.
• Develop and implement marketing initiatives and events.
• Maintain CRM records and track sales activities.
• Educate patients, families, and referrals about hospice care to support informed decisions.
• Collaborate with the Director of Patient Services to project volume and marketing allocations.
• Monitor budgets and allocate resources for marketing activities.
• Provide ongoing training and support to the Care Transition Team.
