Job summary

MBC Talent Connections is seeking a self-motivated Community Liaison for hospice services in San Jose, CA and surrounding areas. This field-based role combines healthcare sales and community outreach to grow referral relationships and expand access to end-of-life care.

You will build trusted partnerships with physicians, discharge planners, and other providers, conduct field visits to drive referrals, and support patients and families with sensitivity and professionalism.

Qualifications

• Bachelor's degree preferred; equivalent experience considered.
• Minimum 2 years healthcare sales or marketing experience required.
• Hospice or palliative care sales experience strongly preferred.

Responsibilities

• Build and maintain trust-based relationships with physicians, hospital discharge planners, SNFs, social workers, and other healthcare professionals.
• Establish and grow a network of referral sources across the San Jose service area.
• Serve as a credible representative of hospice care within the medical and broader healthcare community.
• Conduct regular field visits to drive referral growth and increase hospice adoption.
• Position the organization as the provider of choice in the service area, contributing to census goals.
• Identify new partnership opportunities with community agencies and healthcare organizations.
• Lead initial conversations with patients and families about hospice care, explaining services and philosophy clearly.
• Guide families through consent processes with empathy and professionalism.
