MBC Talent Connections is seeking a self-motivated Community Liaison for hospice services in San Jose, CA and surrounding areas. This field-based role combines healthcare sales and community outreach to grow referral relationships and expand access to end-of-life care.

You will build trusted partnerships with physicians, discharge planners, and other providers, conduct field visits to drive referrals, and support patients and families with sensitivity and professionalism.

Competenze

• Bachelor's degree preferred; equivalent experience considered.
• Minimum 2 years healthcare sales or marketing experience required.
• Hospice or palliative care sales experience strongly preferred.

Community Liaison – Hospice Services

• Monday through Friday, 8:00 AM to 4:30 PM

Overview

A mission-driven hospice care provider serving the greater San Jose area is seeking a self-motivated, compassionate Community Liaison to grow referral relationships and expand access to end-of-life care services. This is a field-based role at the intersection of healthcare sales and community outreach, ideal for someone who thrives on building meaningful connections and making a tangible difference in patients' lives.

What You’ll Do

Relationship Development

• Build and maintain trust-based relationships with physicians, hospital discharge planners, skilled nursing facilities, social workers, and other healthcare professionals
• Establish and grow a network of referral sources across the San Jose service area
• Serve as a knowledgeable, credible representative of hospice care within the medical and broader healthcare community

Outreach and Growth

• Conduct regular, intentional field visits to drive referral growth and increase preference for hospice services
• Position the organization as the provider of choice across the service area, contributing to census goals
• Identify new partnership opportunities with community agencies and healthcare organizations

Patient and Family Support

• Lead initial conversations with patients and families about hospice care, clearly explaining available services and the care philosophy
• Guide families through consent processes with empathy, respect, and professionalism

What You Bring

Experience and Education

• Bachelor's degree in a related field preferred; equivalent experience will be considered
• Minimum 2 years of healthcare sales or marketing experience required
• Hospice or palliative care sales experience strongly preferred
• Prior clinical background is a plus

Skills and Attributes

• Proven ability to build and sustain relationships with physicians, discharge planners, and healthcare partners
• Self-directed and comfortable managing a field territory with minimal supervision
• Strong communication skills and a genuine passion for serving patients and families
• Must have reliable transportation, a valid driver's license, and current auto insurance

Compensation and Benefits

Pay

• $65 to $70 per hour base pay (range; final offer commensurate with experience), plus uncapped commission
