Job summary

Kindful Health is seeking a Hospice Sales Liaison to establish and develop new business contacts, maintain relationships and implement a sales and marketing plan to cover the assigned region.

This role involves communicating with patients, families, physicians, social workers, discharge planners and external vendors, generating referrals and managing CRM records to support admissions and service delivery.

Qualifications

• Bachelor's degree preferred and at least two years of hospice sales experience.
• Proven ability to generate qualified referrals and manage referral sources in CRM.

Responsibilities

• Generates qualified referrals to achieve hospice admissions and meet target goals.
• Maintains up-to-date records of referral sources in the CRM.
• Assists care center with obtaining documentation to complete referrals to admissions.
• Evaluates referrals for suitability for hospice admission.
• Provides consultation to referrers, patients and families about hospice options.
• Coordinates with care team to allocate resources and ensure needed equipment/services.
• Acts as liaison to patients and families during transition from discharge to hospice initiation.
• Develops and maintains relationships with referral sources and institutions.
• Prepares business plans and maintains target account lists; prioritizes accounts.
