Job summary

Choice Hospice, Inc. is seeking a Hospice Field RN in Glendale, AZ to deliver compassionate, patient-centered care in homes and care settings.

You will coordinate comprehensive nursing care plans, collaborate with physicians, social workers, and chaplains, and support families to ensure comfort and dignity at end of life. Requirements include an active RN license, CPR, and a valid driver’s license with auto insurance.

Qualifications

• Current RN license required.
• CPR certification required.
• Valid driver’s license required.
• Auto insurance required.
• Hospice RN experience (2+ years) preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s needs due to terminal illness.
• Apply admission criteria to hospice care to establish levels of care and eligibility.
• Develop and document the plan of care; revise as necessary.
• Educate patient and family on disease, self-care, end-of-life care; coordinate with IDG.
