Job summary

Choice Health At Home is seeking a Hospice Field RN (PRN) in Tucson, AZ to deliver compassionate, patient-centered hospice care in homes and care settings. You will assess needs, coordinate comprehensive nursing plans, and work with the IDG team to ensure comfort, dignity, and family support.

The role requires an active RN license, CPR certification, and a valid driver’s license with available on-call responsibilities. HomeCare HomeBase experience is preferred to optimize care delivery.

Qualifications

• Current RN license is required.
• CPR certification.
• Valid driver’s license.
• Valid auto insurance.
• At least 2 years Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s physical, functional, psychosocial, and environmental needs.
• Apply admission and re-certification criteria for hospice care to establish levels of care and eligibility.
• Develop, document, and revise the plan of care to ensure quality and continuity of care.
