Job summary

Qualifications

• RN licensure in Texas is required.
• Bachelor’s degree in nursing preferred.
• 1+ years of hospice experience or hospital experience required.
• Strong interpersonal skills and ability to communicate with diverse patients and team members.

Responsibilities

• Provides direct nursing case management for hospice patients.
• Coordinates patient plan of care with the multidisciplinary team.
• Documents activities and assessments in the clinical information system.
• Educates and supports patients and families about care expectations and changes in condition.
• Travel to multiple patient locations as part of the role.
