Job summary

Pathways Hospice is seeking an experienced RN Case Manager to support our hospice patients across Northwest, Southwest & Southeast Texas. This PRN, on-call role involves leading the nursing care team and coordinating with physicians and therapy professionals to ensure high-quality patient care.

Responsibilities include assessing patient needs, developing care plans, educating patients and families, and maintaining precise clinical notes.

Qualifications

• Valid RN license in Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HomeCare HomeBase (HCHB) EMR (preferred).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients.
• If working the weekend, will receive the following Monday off (Admissions Included).
• Must be willing to participate in an On Call Rotation (Weekly).
