Harbor Healthcare System in the Houston area is seeking a part-time Registered Nurse Case Manager. This outpatient role requires an independent RN to admit and coordinate care for patients and families, including admissions, assessments, discharges and pronouncements.

The RN-Case Manager works within an interdisciplinary team to ensure appropriate care planning and seamless transitions, with preferred Hospice or Home Health experience and must have Texas RN licensure and reliable transportation.

Qualifikationen

• Texas RN license - required (State of Texas)
• 1+ years' RN experience - preferred
• Hospice or Home Health experience - preferred
• Effective written and verbal communication
• Ability to travel - reliable transportation
• Clean background, drug screen, and driving record

Job Description:

This is an out-patient position,that requires an independent and experienced RN.
