Job summary

Houston Hospice is seeking a PRN - RN Field Case Manager to join our growing team south of I-10 in the Houston area. This role manages patient cases, coordinates care with families and the interdisciplinary team, and ensures documentation in the clinical information system.

We value experienced RNs with hospice or hospital case management background, Texas license, and strong communication in a team-based environment. Travel across assigned locations is required.

Qualifications

• Graduate of an accredited nursing program with current Texas license.
• Bachelor's degree in nursing preferred.
• 1+ years of hospice or hospital case management experience.
• Current Basic Life Support certification.

Responsibilities

• Provides direct nursing care case management for an RN-licensed professional.
• Coordinates care with client, family and multidisciplinary team per plan of care.
• Documents activities and assessments in the clinical information system.
• Educates and supports the patient's family, communicates changes in condition.
• Willingness to travel to multiple work locations daily.
