Job summary

Trinity Hospice is seeking an experienced Registered Nurse to join our clinical team as an RN Case Manager in Houston. You will assess patients and families, develop and manage patient-centered care plans, and coordinate care across an interdisciplinary team to ensure comfort and quality of life.

We value evidence-based practice, strong clinical judgment, and reliable transportation to support home and facility visits.

Qualifications

Current Registered Nurse license in the state where you will be working.
One year of RN Case Manager or hospice experience preferred.
Must have a valid driver's license and reliable transportation.

Responsibilities

Perform comprehensive nursing assessments to identify physical, psychosocial, and emotional needs.
Develop and evaluate individualized, patient-centered care plans focused on comfort and quality of life.
Provide skilled nursing interventions including pain and symptom management and medication administration.
Coordinate care as the primary RN case manager across the interdisciplinary team.
Collaborate with physicians, social workers, chaplains, and hospice aides for seamless care delivery.
