Job summary

Trustbridge Hospice, a member of Empath Health, seeks a dedicated CNA to deliver compassionate in-home care across Palm Beach County. Per diem schedule 7:00am–3:30pm with weekend availability; bilingual English/Spanish encouraged.

We provide full Life Care with competitive pay, comprehensive benefits, and 5+ weeks of PTO. You will work under RN/LPN guidance to support ADLs, mobility, and patient comfort in home settings.

Qualifications

• Current Florida CNA license.
• Current Basic Life Support (BLS) certification.
• At least 6 months of experience in a hospital or healthcare setting.
• Ability to drive and reliable vehicle with valid license and insurance.

Responsibilities

• Provide direct and indirect patient care in home and community-based settings.
• Assist with activities of daily living (ADLs) and comfort measures.
• Maintain a clean, safe, and supportive home environment for patients.
• Offer emotional support to patients and families with dignity and compassion.

What you'll Do

As a CNA on our hospice team, you will deliver high-quality, compassionate care under the guidance of a Registered Nurse (RN) or Licensed Practical Nurse (LPN):

• Provide direct and indirect care to patients in home and community-based settings
• Assist patients with activities of daily living (ADLs), including personal hygiene, mobility, and comfort measures
• Support a clean, safe, and nurturing environment for each patient
• Offer emotional support to patients and their families, fostering dignity and compassion at every interaction

What You'll Need

• Current license to practice as a Certified Nursing Assistant in the State of Florida.
• Current Basic Life Support Certification.
• Six months’ experience working in hospital or healthcare environment
• Requires ability to drive, a reliable car, a good driving record, automobile insurance and basic knowledge of the county territory.
• Must be able to fulfill assigned tasks effectively and efficiently.
• Must practice good personal hygiene and be neat and clean in appearance.
