Magnolia Hospice Granbury is seeking a Hospice Education Specialist to expand access by building relationships with referral sources and educating the community about hospice services.

You will collaborate with clinical teams to ensure smooth transitions from referral to admission, while maintaining HIPAA-compliant communications and accurate CRM records. A patient-centered, compassionate approach is essential.

Qualifications

• Education: High school diploma or equivalent; Bachelor's degree preferred.
• Experience: 1–3 years in sales, marketing or business development; home care or hospice preferred.
• License/Certification: Driver’s license and auto liability insurance; no OIG listing; nurse aide registry compliance.

Responsabilités

• Develop and maintain relationships with referral sources (physicians, hospitals, SNFs, assisted living).
• Represent Magnolia Hospice at health fairs and community events.
• Educate stakeholders on hospice eligibility and care approach; advocate for patient access.
• Collaborate with intake and clinical teams to ensure smooth referral to admission; document in CRM per HIPAA.
