Job summary

Acacia Health seeks an experienced Hospice Community Liaison to expand referrals across Riverside County. You will educate hospitals, SNFs, and physicians about our hospice services, and develop call plans with targeted visits to grow partnerships.

Responsibilities include community education, presenting information to accounts, and coordinating with the Director of Business Development and clinical teams to ensure aligned, compassionate care.

Qualifications

• Bachelor’s Degree preferred.
• Healthcare Sales Professional. Experience in hospice required.
• Two year experience in health care sales and marketing.

Responsibilities

• Educate the community and partners about hospice services.
• Manage accounts within the defined territory including hospitals, skilled nursing facilities, and physicians who refer to Acacia.
• Develop PreCall plans, weekly routing, and visits.
• Prepare and present educational information to community partners.
• Conduct inservices with accounts and coordinate with the Director of Business Development.
