Job summary

Acacia Health seeks an experienced Hospice Community Liaison to expand referrals across Riverside County. You will educate hospitals, SNFs, and physicians about our hospice services, and develop call plans with targeted visits to grow partnerships.

Responsibilities include community education, presenting information to accounts, and coordinating with the Director of Business Development and clinical teams to ensure aligned, compassionate care.

Qualifications

• Bachelor’s Degree preferred.
• Healthcare Sales Professional. Experience in hospice required.
• Two year experience in health care sales and marketing.

Job Duties:

• Responsible for educating the community and our partners regarding our hospice services.
• Manage all accounts within defined territory which include hospitals, skilled nursing facilities, assisted living facilities, physicians and other in a position to refer home health to Acacia.
• Develop PreCall plan/Weekly Routing and visits.
• Developing and presenting educational information to community.
• Prepare and conduct inservices with accounts.
• Reports to the Director of Business Development and works closely with the Clinical Interdisciplinary Group, Intake Coordinator and office staff.
• Other duties as assigned by DBD and leadership team.

Qualifications:

• Bachelor’s Degree preferred.
• Healthcare Sales Professional. Experience in hospice required.
• Two year experience in health care sales and marketing.

• Strong knowledge of the Hospice philosophy, industry, and regulations relevant to job responsibilities.
