Job summary

Acacia Health is seeking an experienced Registered Nurse to lead hospice care in patients' homes and communities. You will plan, organize, and direct hospice services, coordinating care for patients and families in Inland Empire, Jurupa Valley and surrounding cities.

The role requires a California RN license, CPR/BLS certification, and a valid CA driver license, with hospice experience preferred. Field-based, compassionate care is essential to this position.

Qualifications

• CA RN License required.
• CPR/BLS certification required.
• CA Driver License required.
• Hospice: 1 year of experience required.

• Experience with HomecareHomebase (Pointcare) preferred.

Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and comfort care.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Teaches the patient and family/caregiver self-care techniques as appropriate.
• Attends interdisciplinary group meetings.

Job Description Summary

• Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
