Community Healthcare of Texas is seeking a Community Marketing Liaison to promote the hospice program to physicians, discharge planners, and other referral sources. The role acts as a bridge between hospice and the medical community to improve referrals and awareness.

The position also involves coordinating education programs, maintaining community visibility, and tracking interactions with referral sources to ensure high-quality care coordination for patients and families.

Qualifications

• College degree or equivalent experience is required.
• 2+ years of marketing experience.
• Hospice or Home Health experience preferred.
• Proficient with computers and software.
• Ability to build relationships and communicate clearly.

Responsibilities

• Markets the hospice program to physicians and discharge planners; promotes hospice to referral sources.
• Maintains community visibility and attends networking programs.
• Coordinates education programs in hospitals and clinics.
• Tracks and reports monthly contacts with referral sources.
