Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to manage relationships with referral sources and medical partners. You will educate stakeholders about Enhabit services and program requirements to grow referrals and support smooth post-acute transitions.

The role emphasizes clinical and regulatory knowledge, strong interpersonal skills, and reliable transportation for in-person engagements, with a focus on improving patient outcomes and satisfaction.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or graduate of nursing/therapy/social work; equivalent experience considered.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of Medicare/home health regulations and licensing requirements.

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources (physicians, hospitals, senior living communities, etc.).
• Educate referral sources and families about Enhabit services and program requirements to drive referrals.
• Support post-acute care transitions to improve patient outcomes and satisfaction.
