Job summary

The Care Team Hospice is seeking a driven Community Care Liaison to strengthen our presence in the community and grow census through partnerships with physicians, hospitals, and referral sources. You will educate providers, patients, and families on hospice value and guide them toward high-quality care.

This role involves regular travel within the assigned geography, building trust, and serving as the face of hospice services while collaborating with a supportive team and comprehensive benefits

Qualifications

• Bachelor’s degree or equivalent.
• 1-2 years healthcare outside sales experience.
• 1-2 years Hospice or Home Health sales preferred.
• Ability to travel within assigned geography and beyond.
• Valid driver's license and automobile insurance.

Responsibilities

• Build and expand relationships with physicians, hospitals, and other referral sources to drive referral volume and market growth.
• Educate the community and share the value of comfort-focused, patient-centered care.
• Achieve and exceed census and admission targets by positioning hospice as a preferred provider.
• Engage regularly with providers and referral sources to create strong, collaborative partnerships.
• Lead discussions with patients and families, communicating services with professionalism and empathy.
