Job summary

The Care Team Hospice seeks a driven Community Care Liaison to grow our presence in the community. You will build relationships with physicians, hospitals, and referral sources to increase awareness and utilization of our hospice services.

You will educate providers, patients, and families on hospice value and guide them toward compassionate, high-quality care while meeting census targets through strong partnerships.

Qualifications

• Bachelor’s degree or equivalent required.
• 1-2 years healthcare outside sales experience.
• 1-2 years Hospice or Home Health sales preferred.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Serve as an advocate for hospice by educating the community and sharing the value of comfort-centered, patient-focused care.
• Achieve census and admission targets by positioning our hospice as a preferred provider within the community.
• Engage with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, communicating services clearly and guiding the consent process with professionalism.
