Job summary

Center4Hospice is seeking a compassionate CNA for the inpatient unit in Mishawaka, IN. This full-time, day-shift role supports patients and families, following a care plan under RN supervision.

You’ll provide direct patient care, assist with ADLs, and help maintain a calm, comforting environment. The position emphasizes independence within an interdisciplinary team, adherence to documentation, and compassionate communication with patients and families.

Qualifications

• Current Indiana CNA license.
• Michigan CNA license is a plus.
• Ability to work independently while collaborating with an interdisciplinary team.
• Strong clinical assessment, communication and critical-thinking skills.
• Hospice or inpatient care experience preferred.

Responsibilities

• Provides care and services to patients according to the care plan or as approved by the RN.
• Assists with activities of daily living: bed making, meals, and light cleaning.
• Offers support and encouragement to patients and families.
• Demonstrates proper body mechanics during patient care.
• Reports significant changes to the RN and updates scheduling or patient incidents.
