Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide personal and comfort-focused care to hospice patients in homes and facility settings. You will collaborate with an interdisciplinary team to uphold dignity, compassion, and quality of life throughout the hospice journey.

Responsibilities include assisting with daily living activities, reporting patient condition changes, and timely EMR documentation using HCHB.

Qualifications

• Active CNA certification.
• Hospice aide experience preferred.
• Experience using HCHB (Homecare Homebase) preferred.

The Care Team Hospice is seeking a CNA to provide personal care and comfort-focused support to hospice patients in their homes and facility settings while working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Schedule, Hours, Territory, & Pay

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Up to a 1-hour radius around Mishawaka, IN
• $18-22/hr. plus mileage reimbursement

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
