Center4Hospice is seeking a Hospice Certified Nurse (CNA) for a Saturday-Sunday day shift in Mishawaka, IN. You’ll care for patients, build meaningful relationships with families, and use clinical judgment to keep patients comfortable and dignified.

Qualifications

• Indiana CNA license is required.
• Michigan CNA license is a plus.

• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Provides care and services to patients according to the care plan or as approved by the RN.
• Assists with activities of daily living: bed making, meal prep, light cleaning.
• Offers support and encouragement to patients and families.
• Demonstrates proper body mechanics during care tasks.
• Reports changes in patient condition or scheduling to RN.
• Maintains daily documentation of patient care.
