You will build trusted relationships with patients and families, document care and participate in a structured onboarding process in a hospice setting.

The role emphasizes autonomy, strong clinical judgment, teamwork, and the ability to manage a caseload while maintaining timely documentation and clear communication with the care

Qualifications

• Current Indiana CNA license.
• Michigan CNA license is a plus.
• Strong clinical assessment, communication and critical-thinking skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.

Responsibilities

• Provides care according to the care plan or as approved by the RN.
• Assists with activities of daily living.
• Maintains proper body mechanics during patient care.
• Follows RN direction and reports changes.
• Reports significant changes in patient condition or incidents.
