Job summary

You will verify the Plan of Care before visits, assist with patient care, document medication updates, and communicate with the RN Case Manager. Requires a HS diploma or GED, 1 year hospice/NA experience, 50% travel, and reliable

Qualifications

• High School Diploma or GED is required.
• 1 year of experience as a Hospice Aide or Nursing Assistant is required.
• State-established Hospice Aide training program meeting 42 CFR 484.36 and a competency evaluation is required.
• Experience in a home health environment is preferred.
• 50% travel required.
• Valid driver’s license, insurance, and reliable transportation are required.

Responsibilities

• Verifies the Plan of Care prior to each visit and provides care according to RN delegation and physician orders.
• Delivers patient care and documents nursing notes per standards.
• Communicates significant findings to the RN Case Manager and other staff.
• Reports unsafe conditions to the RN Case Manager and physician as appropriate.
• Implements the plan for patient safety using family and community resources.
• Assists the patient in learning self-care techniques while delivering quality care.
• Documents medication regimen and updates the medication profile.
• Collaborates with the RN Case Manager during business hours for guidance.
