Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to provide skilled nursing care in patients’ homes across the Tulsa area. You’ll work under an RN and collaborate with the care team to update the Plan of Care as needed.

The role requires a High School Diploma or GED, 1 year of hospice/NA experience, and formal Hospice Aide training with competency evaluation. Must be able to lift 50 pounds and travel between patient homes.

Qualifications

• High School Diploma or GED is required.
• 1 year of experience as a Hospice Aide or Nursing Assistant is required.
• State-established Hospice Aide training program and competency evaluation.
• Ability to lift, push, pull, and transfer up to 50 pounds; travel between patient homes.
• Home health experience is preferred.

Responsibilities

• Provide skilled nursing care according to the Plan of Care and RN direction.
• Monitor patient condition and report changes to the RN Case Manager and care team.
• Educate patients and caregivers on self-care techniques to support safety and independence.
• Document medications and maintain medication profiles.
