Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to join our team in Enid, OK. You will deliver skilled nursing care per the Plan of Care, report changes to the RN Case Manager, and collaborate with the care team to optimize patient outcomes.

This role emphasizes compassionate, high-quality care delivered in patients' homes. Requirements include a high school diploma or GED, 1 year of hospice aide/NA experience, and a valid driver’s license or reliable transportation to

Qualifications

High School Diploma or GED equivalent required.
1 year of Hospice Aide or Nursing Assistant experience required.
State-established Hospice Aide training program per 42 CFR 484.36 & competency evaluation.
Ability to frequently lift, push, pull up to 50 pounds.
Must have a valid driver's license or reliable transportation for travel to patient homes.

Responsibilities

Provide skilled nursing care per Plan of Care and RN direction.
Monitor patient condition and report changes to RN Case Manager and care team.
Collaborate with RN Case Manager to update the Plan of Care as needed.
Educate patients and caregivers on self-care techniques.
Document medications and maintain accurate medication profiles.
Help prevent avoidable hospitalizations through proactive patient care.
Provide compassionate care with positive communication with patients and families.
