Job summary

Elara Caring in Enid, OK is seeking a Hospice Certified Nurse Assistant/Hospice Aide to provide skilled nursing care per Plan of Care under RN direction. You will educate patients and families, document medications, and help prevent hospital visits during home health visits.

Requirements include a HS diploma or GED, 1 year hospice/NA experience, and training per 42 CFR §484.36 with competency evaluation. Willingness to travel between patient homes and lift up to 50 pounds.

Qualifications

• High School Diploma or GED required.
• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-established Hospice Aide training per 42 CFR §484.36 and competency evaluation program.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes.
• Home health environment experience preferred.

Responsibilities

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and update the Plan of Care as appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care with positive communication to patients, families, physicians, and the care team.
