Elara Caring seeks a Certified Nurse Aide to deliver high-quality home-based patient care under the supervision of a Registered Nurse. You will assist with activities of daily living, monitor vital signs, and support safe mobility in patients' homes.

Responsibilities include following care plans, reporting changes to the team, and maintaining documentation while ensuring patient privacy and safety. A valid CNA/HHA certification and reliable transportation are required.

Formación

• Requires current Home Health Aide or Certified Nursing Assistant certification.
• Ability to frequently lift, push, pull, and transfer patients up to 50 pounds.
• Must have a valid driver's license or reliable transportation to travel between patient homes.

Responsabilidades

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and daily activities.
• Take and record vital signs (temperature, pulse, respirations, blood pressure).
• Assist patients with mobility and safe transfers.
• Follow individualized care plans and report changes to the care team.
• Maintain a clean, safe home environment and protect patient privacy.
