Job summary

Bristol Hospice is seeking an experienced Certified Home Health Aide (CHHA) to provide compassionate in-home care under RN supervision. You will support patients with daily living activities, maintain safety, and communicate with families with empathy and discretion.

Ideal candidates hold CNA/HHA credentials, CPR, and a drive-ready vehicle. Hospice experience is preferred. Join a team committed to dignity, respect, and high-quality end-of-life care.

Qualifications

• Must be a CNA in good standing and meet state requirements.
• Preferred 1 year of hospice or community health experience.
• Valid driver’s license with reliable insured vehicle.
• Current CPR certification is required.
• Ability to follow care plans and document care accurately.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family respectfully and confidentially.
• Assess patient condition and report changes to Case Manager.
• Assist with meals, bathing, grooming, and mobility as directed.
• Maintain a clean, safe living area and follow care procedures.
