Job summary

Bristol Hospice is seeking a compassionate Certified Home Health Aide (CHHA) to join our interdisciplinary team. You will work under the supervision of a Registered Nurse to provide personal care, meals, and comfort, while maintaining patient dignity and privacy in the home setting.

Ideal candidates are CNAs with HHA certification and at least 1 year of community health or hospice exposure. Travel and driving requirements apply; background checks are completed before hire.

Qualifications

• Must be a Certified Nursing Assistant in good standing.
• California residents require CA HHA certification in good standing.
• Preferred 1 year experience in community health/hospice or medical/surgical settings.
• Must have completed 75 hours of classroom and supervised practical training.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Provide personal care to patients after assigned tasks and relay issues to Case Manager promptly.
• Maintain patient safety, monitor and report changes to the Case Manager.
• Assist with meals, bathing, dressing, mobility, and continence as assigned.
• Maintain clean and orderly living areas and adhere to care procedures and documentation standards.
