Job summary

Bristol Hospice is seeking an experienced Certified Home Health Aide (CHHA) to support patients in their homes under the supervision of a Registered Nurse. You will provide compassionate personal care, assist with daily activities, and maintain patient comfort while respecting dignity and privacy.

As an integral member of the Interdisciplinary Group Team, you will document care, communicate planned approaches to families, and adhere to hospice protocols.

Qualifications

• Must be a Certified Nursing Assistant in good standing with a current Home Health Aide certification in CA.
• One year of community health, hospice, or medical/surgical experience preferred.
• Licensed driver with insured vehicle and background check clearance required.

Responsibilities

• Provide personal care such as bathing and grooming and attend to patient requests promptly.
• Maintain confidentiality and communicate clearly with patients, families, and Case Manager.
• Assist with meals, mobility, medications reminders, and basic monitoring as directed by the hospice nurse.
• Document care provided and report significant changes to the Case Manager.
• Maintain a safe, clean living environment and follow care procedures and standards.
