Job summary

Advocate Health seeks a Hospice Aide to support patient care in the Hospice North Region. The role requires weekend on-call rotations, a 40-hour workweek, and 8am-5pm shifts with on-call coverage during holidays and summers.

Responsibilities include assisting with daily living activities, reporting patient changes to the RN, and maintaining documentation in accordance with regulatory standards. Certification as a nurse assistant and BLS are required or obtained within 6 months.

Qualifications

• Must be certified as a nurse assistant in the state where the teammate practices.
• BLS for Healthcare Providers certification within 6 months.

Responsibilities

• Provide competent patient care as assigned by RN and plan of care.
• Perform personal care tasks including feeding, laundry, and room cleanliness.
• Record documentation accurately and maintain patient confidentiality (HIPAA).
• Assist with patient positioning, vital signs, and basic monitoring as directed by RN.
