Zusammenfassung

Suncoast Hospice, a member of Empath Health, is seeking a compassionate CNA to provide personal care and daily living assistance for home-based patients in North Pinellas County. The position is Monday–Friday, 8am–5pm, with a focus on compassionate, patient-centered care.

You will document changes and report needs to the registered nurse, and assist families during end-of-life care while pursuing CEU opportunities and organizational PTO benefits.

Qualifikationen

• Valid Florida Board of Nursing certification (CNA License).
• Valid Florida State Driver's License.
• Experience: One to three months related experience and/or training preferred.

• Assist patients with taking medications that are ordinarily self-administered and pre-poured as designated in the Plan of Care.

Suncoast Hospice , a member of Empath Health, is seeking a caring and motivated CNA to provide personal care and tasks related to the activities of daily living for patients in their homes in North Pinellas County. Monday - Friday 8am to 5pm.

What you'll Do

• The provision of hands-on personal care and assistance with activities of daily living.
• Assisting patients with taking medications that are ordinarily self-administered and have been pre-poured as designated in the Plan of Care.
• Hospice Aides must report changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care. Hospice Aide must document the changes and to whom they were reported to.
• Following a patient's death collaborates with primary RN/Counselor and participates as needed in closure contact with family/caregivers.

What You'll Need

• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
