Job summary

Suncoast Hospice, a member of Empath Health, is seeking a caring Hospice CNA to provide personal care and daily living assistance for patients in skilled nursing and assisted living facilities in the Seminole/Largo area. This is a Monday-Friday, 8:00 AM-5:00 PM field-based position.

As part of Empath Health, you will report to the RN, document changes, assist with prescribed medications, and support families during end-of-life care.

Qualifications

• Valid Florida CNA certification required.
• 1–3 months related experience preferred.
• Valid Florida driver's license and reliable transportation required.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living.
• Assist with medications that are self-administered and pre-poured as designated in the Plan of Care.
• Report changes in patient condition to RN/Counselor and document per plan of care.
• Participate in closure contact with family/caregivers after patient death as needed.

Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated Hospice CNA to provide personal care and tasks related to the activities of daily living for hospice patients residing in skilled nursing and assisted living facilities throughout the Seminole/Largo area. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way.What you'll DoThe provision of hands-on personal care and assistance with activities of daily living.Assisting patients with taking medications that are ordinarily self-administered and have been pre-poured as designated in the Plan of Care.Hospice Aides must report changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care.
