Job summary

Suncoast Hospice, a member of Empath Health, seeks a caring Hospice CNA to provide personal care and assist with daily living activities for patients in skilled nursing and assisted living facilities across the Seminole/Largo area.

This field-based role runs Monday–Friday, 8:00 AM–5:00 PM. A Florida CNA certification and a valid driver’s license are required to perform patient visits with reliable transportation available.

Qualifications

• Valid Florida CNA certification (CNA license).
• Experience in hospice or home health care preferred.
• Ability to lift, transfer, and assist with ADLs.
• Strong communication with patients, families, and care team.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living for hospice patients.
• Assist patients with medications self-administered and pre-poured per Plan of Care.
• Report changes in patient conditions to RN/Counselor and document care updates.
• Support families during patient death and participate in closure activities as needed.

Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated Hospice CNA to provide personal care and tasks related to the activities of daily living for hospice patients residing in skilled nursing and assisted living facilities throughout the Seminole/Largo area.

This is a Monday-Friday, 8:00 AM-5:00 PM field-based position.

What you'll Do

• The provision of hands-on personal care and assistance with activities of daily living.
• Assisting patients with taking medications that are ordinarily self-administered and have been pre-poured as designated in the Plan of Care.
• Hospice Aides must report changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care.
• Hospice Aide must document the changes and to whom they were reported to.
• Following a patient's death collaborates with primary RN/Counselor and participates as needed in closure contact with family/caregivers.

What You’ll Need

• One to three months related experience and/or training preferred.
• Valid Florida Board of Nursing certification (CNA License).
• Valid Florida State Driver's License For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
