Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to provide hands-on personal care for patients in their homes, facilities, or assisted living settings. You will work with an RN Case Manager, ensuring comfort, dignity, and compassionate care in the final chapter of life.

The role emphasizes teamwork, clear expectations, and manageable caseloads, with opportunities to grow within a supportive, collaborative team and a fleet car program.

Qualifications

• Active CNA certification is required.
• Healthcare caregiving experience in home care, assisted living, or hospice is a plus.
• Current CPR certification preferred.
• Strong written, verbal, and listening communication are essential.
• Ability to manage time and handle emotional situations with care.

Responsibilities

• Provide personal care including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under the RN Case Manager.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.
