Job summary
Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care for patients in home, assisted living, or care facilities. You will work with a dedicated RN Case Manager within an interdisciplinary team, focusing on comfort, dignity, and compassionate care in the final chapter of life.
The role includes a fleet car program, company cell phone, and comprehensive benefits.
Qualifications
- Active CNA certification (required).
- Healthcare caregiving experience in home care, assisted living, or hospice a plus.
- Current CPR certification (preferred).
- Ability to work in an electronic medical record system.
- Strong communication skills, including written and verbal.
- Compassion, patience, and ability to handle emotional situations with care.
- Ability to lift 50 lbs and maintain safe body mechanics.
Responsibilities
- Provide personal care with activities of daily living (bathing, dressing, grooming, toileting, skin care).
- Assist with lifting, transferring, positioning, ROM, exercise, and ambulation.
- Assist with dining and nutrition.
- Review and follow each patient’s plan of care under RN supervision.
- Provide comfort and companionship to patients and support to families.
- Maintain electronic medical records.
