Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care and support to patients in home or facility settings. You will work under an RN Case Manager, following patient care plans, with a focus on comfort, cleanliness, and dignity in end-of-life care.

The role includes lifting, positioning, daily living activities, and documentation in electronic medical records.

Qualifications

• Active Certified Nursing Assistant (CNA) certification is required.
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice is a plus.
• Current CPR certification is preferred.
• Ability to work in an electronic medical record system.
• Strong communication skills, including written, verbal, and active listening.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and the ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and good understanding of body mechanics.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
