Job summary
Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide for full-time field-based work. You will provide hands-on personal care to patients in home, assisted living, or hospice settings, ensuring comfort, cleanliness and dignity.
Work closely with an RN Case Manager within an interdisciplinary team, with manageable caseloads and structured training to grow your nursing career. Competitive hourly pay and a fleet car program support your role, along with benefits such as
Qualifications
- Active Certified Nursing Assistant (CNA) certification is required.
- Experience in home care, assisted living, memory care, skilled nursing, hospital, or hospice is a plus.
- CPR certification is preferred.
- Ability to work in an electronic medical record system.
- Strong communication, written and verbal skills; active listening is valued.
Responsibilities
- Provide personal care as part of activities of daily living (bathing, dressing, grooming, toileting, skin care).
- Assist with lifting, transferring, positioning, ROM, exercise and ambulation.
- Assist with dining and nutrition.
- Review and follow each patient’s plan of care under RN guidance.
- Provide comfort and companionship to patients and support to families.
- Maintain electronic medical records.
