The role requires an RN with hospice/home health experience, strong IDT oversight, and a track record of improving quality and efficiency in a clinician-owned hospice setting.

Qualifikationen

• Active and unrestricted RN license in New Jersey
• 2+ years hospice, home health, or palliative care experience
• 1+ year of clinical leadership or supervisory experience
• Strong knowledge of hospice regulations including CMS/CHAP requirements
• End-of-life care and pain and symptom management understanding
• Care planning, documentation compliance, and IDT processes knowledge
• Experience with staff supervision, performance management, and team leadership
• Excellent clinical decision-making, crisis intervention, and family communication
• Valid driver’s license, reliable transportation, and auto insurance

Aufgaben

• Oversee clinical operations and patient care services within hospice programs
• Lead Interdisciplinary Team (IDT) meetings for care planning
• Supervise and mentor RNs, LPNs, CHHAs, social workers and staff
• Ensure documentation complies with CMS, CHAP and NJ regulations
• Monitor Medicare Conditions of Participation compliance
• Collaborate with Compliance to develop clinical education and training
• Conduct facility visits to strengthen partnerships
• Oversee staffing, scheduling, payroll, mileage reimbursement and supplies
• Drive quality improvement and patient satisfaction initiatives
