Job summary

Advanced Home Health and Hospice is seeking a Hospice Clinical Director to oversee clinical operations, ensure regulatory compliance, and foster a compassionate, excellence-driven culture in end-of-life care. The role involves leading nursing staff, developing policies, and collaborating with medical directors to optimize patient plans.

You will conduct audits, provide ongoing staff education, and liaison with families and regulatory bodies to ensure top-quality care.

Qualifications

• Current Registered Nurse (RN) license in the state where work is performed.
• EMR experience with Matrix Care.
• Minimum of 1 year of clinical experience in hospice or palliative care.
• At least 1 year of experience in healthcare leadership.
• At least 1 year of experience in case management.

Responsibilities

• Oversee daily hospice operations and ensure high-quality patient care.
• Lead and mentor nursing and clinical staff with guidance on patient care practices.
• Develop, implement, and evaluate clinical policies to improve care quality.
• Collaborate with medical directors and care teams to plan and evaluate patient care plans.
• Conduct clinical audits to ensure adherence to standards and best practices.
• Provide ongoing education and training for clinical staff on new policies and care techniques.
• Serve as liaison with regulatory agencies, healthcare providers, and families.
• Address patient and family concerns with effective communication and issue resolution.
• Stay current with home health care regulations and industry trends.
• Ensure the agency runs smoothly in coordination with the Administrator.
• Ensure accurate documentation of care, medications, and times for insurance and quality purposes.
• Provide supervision and feedback to nursing staff for growth and development.
• Oversee recruitment, onboarding, and retention of nursing staff.
