Hospice Austin seeks a Clinical Team Coordinator to support daily clinical operations, staff coordination, and workflow management to ensure the delivery of high-quality hospice care for patients and families. Qualifications

• Graduation from an approved school of professional nursing as determined by the Board of Nurse Examiners.
• Unrestricted license as a Registered Nurse by the Texas Board of Nurse Examiners.
• Valid Texas driver’s license, auto liability insurance and safe driving record.
• Knowledge of hospice federal and state regulations.
• Strong computer skills including knowledge of and experience with Microsoft Office products and experience with electronic medical records (EMR).
• CPR certification.
• Strong clinical, communication, and clinical skills.
• Ability to manage multiple priorities in a dynamic, fast-paced environment.
• Compassionate, patient-centered approach with a focus on holistic care.
• Ability to handle emotional and difficult situations with professionalism and empathy.

Responsibilities

• Clinical Oversight & Patient Care: Morning meetings, census review, staffing coverage, safety concerns, and postings.
• Coordinate tasks and ensure timely follow-up with field staff and Team Leads.
• Support IDG meetings with materials and updates; assist Team Lead.
• Coordinate patient transfers to ensure orders and transport steps.
• Monitor Level of Care and ensure HIPAA-compliant documentation.
