Affinity Health Management seeks a Clinical Manager to coordinate patient care in hospice/home care and supervise clinical personnel.

You will review referrals, guide clinicians, and ensure services are delivered with high quality, while maintaining compliance and fostering teamwork.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• BSN preferred or 1 year as a professional nurse within the last 3 years.
• Experience in hospice/home care and 2 years management or supervisory experience.
• Complies with accepted professional standards and practice.
• HCHB and PointCare experience preferred.
• Excellent observation, nursing judgment, and communication skills.

Responsibilities

• Receives case referrals and assigns hospice personnel; conferences with Medical Director on eligibility.
• Reviews and evaluates cases via home visits, conferences, record review and clinician services; guides clinicians.
• Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course.
• Performs medical records management and workstation in EMR system.
• Assists clinicians in establishing goals, setting priorities, and developing plan of care.
• Attends case conferences to coordinate care and interdisciplinary discussions.
• Conducts quarterly record reviews and reports findings to Clinical Director and hospice staff.
• Assists in screening and interviewing new employees and advising on employment; supports orientation.
• Plans, implements and evaluates inservice and continuing education programs; assists in policy development.
• Complies with professional standards and practices.
• Assesses community health needs and helps develop plans to meet them; participates in public relations and outreach.
• Participates in quality improvement teams and activities.
• Performs other duties as delegated by the Clinical Director/Director of Patient Care Services.
