Job summary

Affinity Health Management is seeking a Clinical Manager to ensure coordinated patient care in hospice/home care. The role supervises clinical staff and collaborates with the Medical Director on eligibility decisions.

You will drive care plans, supervise documentation in EMR, and participate in quality improvement initiatives. The ideal candidate is a registered nurse with leadership experience in hospice/home care, familiar with HCHB and PointCare, and committed to high‑standards of patient

Qualifications

• Registered nurse with current Florida RN license.
• BSN preferred or minimum 1 year of professional nursing in last 3 years.
• Experience in hospice/home care and 2 years of management or supervision.
• Adheres to professional standards and practices.
• HCHB and PointCare experience preferred.

Responsibilities

• Receive case referrals and review patient information to determine hospice/home care needs and assign staff.
• Review and guide cases via home visits, conferences, and record review to improve care delivery.
• Assess medical diagnoses, prognosis, medications, and clinical course.
• Manage medical records and EMR workflow.
• Assist clinicians in setting immediate and long‑term care goals and plans.
• Attend case conferences to coordinate care and interdisciplinary involvement.
• Conduct quarterly record reviews and report findings to leadership and staff.
• Assist in screening and interviewing new employees and orient them.
• Support training programs and assist in policy development and policy implementation.
• Ensure compliance with professional standards and promote quality improvement.
• Assess community health needs and develop programs to meet them; participate in public relations.
• Participate in quality assessment and performance improvement activities.
