Affinity Hospice Holdings, LLC seeks a Clinical Manager to oversee the coordination and management of hospice clinical services. The role directs patient care, ensures compliance, and drives quality outcomes across the clinical team.

Qualifications

• Must hold RN license in the state where the agency is located.
• Bachelor's degree in Nursing from an NLN-accredited program.
• Three years of leadership/management experience in hospice or related healthcare.
• Two years of direct hospice field experience.
• Demonstrated ability to supervise and direct professional administrative personnel.
• Must have valid driver's license, reliable transportation, and auto liability insurance.
• Able to lift 40 lbs from floor to waist, 10 lbs waist to shoulder, and 40 lbs to carry/pull.
• Able to assist with patient lifts and ensure staffing is clinically appropriate.

Responsibilities

• Coordinate, deliver, and manage patient care across the hospice clinical team.
• Interpret indicators to adjust staffing levels, revenues, or expenses.
• Hire, evaluate, and terminate clinical personnel as needed.
• Ensure all clinical notes, orders, and processes meet policy and compliance.
• Provide new employee orientation and ongoing staff education.
• Support QA programs, performance improvement, and regulatory audits.
• Coordinate on-call, weekend, and vacation schedules for clinical staff.
• Ensure care delivery complies with licensure, state/federal laws, and ACHC/JCAHO standards.
• Promote hospice referrals within the healthcare community.
