Job summary

Affinity Hospice Management LLC is seeking a Clinical Manager to ensure coordinated patient care and supervision of clinical staff. This role emphasizes liaison with the Medical Director, case management, and EMR workflow optimization.

The ideal candidate brings active RN licensure, hospice/home care experience, and 2+ years of supervisory responsibilities, with strong communication and adherence to professional standards. A driver and CPR certification are required.

Qualifications

• Current RN license in the state.
• BSN preferred or 1 year RN experience in last 3 years.
• Hospice/home care experience with 2+ years in management/supervision.
• Adheres to professional standards and practice.
• HCHB and PointCare experience preferred.
• Excellent observation, nursing judgment and communication skills.
• Understands hospice philosophy and pain/symptom management.
• Must have a licensed driver with insured vehicle.
• CPR certification required.
• Demonstrates verbal and written communication skills.

Responsibilities

• Coordinate patient care and supervise clinical staff.
• Review cases through home visits, conferences and chart reviews to guide clinicians.
• Consult with Medical Director on patient eligibility and plan of care.
• Manage medical records and EMR workflow.
• Lead case conferences to discuss interdisciplinary care and coordination.
• Assist in staff screening, interviewing and orientation of new employees.
• Help develop and evaluate policies, procedures and organizational goals.
• Support quality improvement initiatives and community outreach.
