Qualifications

• Registered nurse with current licensure to practice in California.
• BSN degree preferred.
• Two years of management or supervisory experience in health care, preferably home care.
• Maintains CPR certification.
• Demonstrates excellent observation, verbal and written communication skills.

Responsibilities

• Receives case referrals and assigns clinicians to cases.
• Reviews services provided and guides clinicians to improve performance.
• Reviews patient diagnoses, medications, procedures and clinical course.
• Sets priorities and develops plans of care with clinicians.
• Attends case conferences to coordinate care.
• Conducts quarterly record reviews and communicates findings to leadership.
• Assists in interviewing and orientation of new staff.
• Supports planning and evaluation of in-service and continuing education programs.
